Attention Pharmacist Candidates;
The evaluation results of the oral exam held on 03/05/2023 and 04/05/2023 with candidates who applied for the pharmacist position at our Hospitals are presented below for your information. The list of documents requested from the personnel to be placed according to the oral exam evaluation result is included. The documents must be completed and submitted to the Human Resources Coordinatorship (Adult Hospital, C Floor) by the end of working hours on 18/05/2023. Documents Required from Candidates Who Will Start Work
• 4 passport-size photographs
• Health Board Report (a report from a 5-signature board stating that there is no obstacle to working)
• 2 photocopies of the identity document
• Residence Certificate (a document obtained via e-Devlet is valid).
• Criminal Record Certificate (a document obtained via e-Devlet is valid).
• The original and 2 photocopies of the higher education diploma required in the announcement.
• A document from the institution where the candidate worked within the last year showing their employment status
• SGK Registration and Service Record (requested if the candidate has worked before.)
• For male candidates, the original and a photocopy of the military discharge certificate or deferment certificate (a document obtained via e-Devlet is valid).
